Annual health checks if you have a learning disability

If you are aged 14 and over, have a learning disability and are on your doctor’s learning disability register, you should be invited to an annual health check.

If you haven’t been invited but are aged 14+ you can ring your GP practice to arrange or ask your parent / carer to book one for you.

What is an annual health check?

An annual health check is when a doctor checks how healthy you are. This usually happens once a year.

Your LD Annual Health Check is a chance to meet your GP/ Practice nurse or Health Care Assistant and have your health monitored.

It is good to go to the GP practice when you are well so that when you are poorly you feel comfortable attending an appointment as you know the people and environment.

If you are worried about going to an annual health check, speak to your doctor or nurse and they can make changes to help you. This is called making reasonable adjustments.

What will happen at an annual health check?

You can ask someone to come with you for your annual health check. The doctor will ask how you are, what medication you are taking, how you are feeling and can help you make a health action plan. A health action plan is a plan where you can write down the things you can do to be healthy.
