- You will need to complete a separate form for each team in your club
- Any forms received with missing information will be returned, which will delay your request
- Forms requesting no changes are allocated first
- Any changes to league, ground, age group or time slot are allocated second and may affect the continued use of your current allocation
- New applications are considered third.
